Acknowledgment of Risk

I acknowledge and agree that participation in the "Cycle for MS" and/or "Yoga for MS" program involves physical activity that carries inherent risks, including but not limited to personal injury, property damage, or other potential hazards. I voluntarily choose to participate, fully understanding and accepting these risks. If I am booking on behalf of multiple participants, I confirm that I have their consent and acknowledge that this waiver applies to all individuals included in the booking.

Release and Waiver

In consideration of my participation in the "Cycle for MS" and/or "Yoga for MS" program, I hereby release, waive, and discharge MS-Take Charge from any and all liability, claims, demands, or causes of action that may arise from my participation. This includes but is not limited to liability for personal injury, illness, death, or property damage, whether caused by negligence or otherwise.

Medical Clearance

I confirm that I am physically capable of participating in the "Cycle for MS" and/or "Yoga for MS" program and that I have consulted with a healthcare professional if necessary. I understand that it is my responsibility to monitor my own physical condition during participation and to discontinue activity if any discomfort or health concerns arise.

 

Indemnification

I agree to indemnify and hold harmless MS-Take Charge from any claims, liabilities, damages, costs, or expenses (including attorney’s fees) arising from my participation in the "Cycle for MS" and/or "Yoga for MS" program.

Consent to Emergency Treatment

In the event of a medical emergency, I authorize MS-Take Charge to seek medical treatment on my behalf and agree to assume full responsibility for any associated costs.

Media Release

I grant permission for MS-Take Charge to use any photographs, videos, or other media of me taken during participation for promotional or educational purposes without compensation.
